Are filtered shower heads worth it depends almost entirely on what is wrong with your water. If the complaint is chlorine smell, dry skin after washing or a chemical odor in the steam, they do a genuine job for very little money. If the complaint is hard water, limescale on the glass or hair that will not rinse clean, they will disappoint you.
What the media inside can do
Most filtered shower heads use one of two media, often together. KDF is a copper-zinc alloy that reduces chlorine through a redox reaction and works at the high temperatures a shower runs at, which ordinary carbon handles poorly. Activated carbon adsorbs chlorine and organic compounds but loses effectiveness in hot water, so a carbon-only unit under a hot shower is weaker than the packaging suggests. Some cartridges add a calcium sulfite stage, which reacts with chlorine directly. Against chlorine and its associated smell, these do measurable work in a short contact time.
What they cannot do
Hardness is the honest limitation. The USGS defines hardness as dissolved calcium and magnesium, with water classed as hard from 121 to 180 mg/L as calcium carbonate and very hard above 180 mg/L. Removing that requires ion exchange in a resin bed with a regeneration cycle, and there is no way to fit that into a shower head. A filtered head marketed as softening will not change the hardness number, will not stop scale on the glass, and will not make soap lather. Chloramine is the other weak spot: it is harder to remove than free chlorine and needs longer contact time.
Contact time is the whole game
Any filter's effectiveness depends on how long water spends in contact with the media, and a shower head is a very small cartridge passing a lot of water quickly. That is why the physically larger inline filters that mount on the shower arm ahead of the head generally outperform units that build the media into the head itself. It also explains why performance falls off as the cartridge ages: the media exhausts from the inlet end onward and the effective contact zone shrinks. Change the cartridge on schedule rather than waiting for the smell to come back.

Buy one if you are on a chlorinated municipal supply, you can smell the chlorine in the steam, and your skin or hair reacts after washing. That is a real problem with a cheap and effective fix. Skip it if your problem is scale, spotting or soap that will not lather, because that is hardness and needs a softener on the main line. On a private well with no chlorination there is usually nothing for the media to remove, so the money is better spent on a test.
