A shower hard water filter is the most oversold product in home water treatment. Hardness is dissolved calcium and magnesium, and no cartridge that fits on a shower arm removes it in any lasting way. What the good ones do is real but different: they cut chlorine, catch particles, and in some designs slow the rate at which scale forms on the head.

The USGS describes hardness as dissolved calcium and magnesium, classifying water as soft from 0 to 60 mg/L as calcium carbonate, moderately hard from 61 to 120, hard from 121 to 180, and very hard above 180. Dissolved means the minerals are in solution, not floating as particles, so a sediment screen has nothing to catch and carbon has no affinity for them. Removing hardness needs ion exchange, where the calcium is traded for sodium on a resin bed, or a membrane. Neither of those fits inside a shower cartridge with any useful capacity.
What a shower cartridge with resin can and cannot manage
Some shower filters do include a small quantity of ion exchange resin, and for a short while they genuinely soften the water. The problem is capacity: a bed the size of a shot glass exhausts in a matter of showers on truly hard water, and there is no brine tank to regenerate it, so it stops working long before you replace it. Buyers experience a good first week and gradual disappointment, and conclude the product was fake. It was not fake, it was tiny.

A whole-house softener treats every fixture at full flow with a resin bed large enough to matter and a brine tank to recharge it. That is what changes how the shower feels, stops the scale, and protects the water heater at the same time.
