Water passes over a bed of media whose surface carries nucleation sites. Dissolved hardness minerals gather at those sites and form tiny stable crystals that then detach and travel along with the flow. Because the mineral is already crystallized and suspended, it has little tendency to deposit on a hot element or a shower screen. Nothing is exchanged, nothing regenerates, and no salt or drain connection is involved.
What that gets you and what it does not
Scale protection inside the water heater and the pipework is the honest claim, and it is a real one in the right conditions. What you do not get is soft water: soap will not lather any better, laundry will not soften, spotting on glassware continues because the mineral is still present when the droplet evaporates, and a hardness test on the treated water reads exactly the same as it did before treatment.
