A post carbon filter is the small polishing cartridge fitted at the very end of a treatment system, after everything else and usually after a storage tank. It is the smallest cartridge in most systems and the one that most directly determines what the water tastes like, because it is the last thing the water touches before the tap.

Why storage needs correcting

Reverse osmosis water is stored in a pressure tank with a butyl bladder, often for days, and stored water picks up a faint taste from the bladder material and from the tubing. It is not harmful and it is not a fault, but it is enough to make good water taste slightly plastic or stale. The post filter sits between tank and faucet so that every glass passes through fresh carbon on the way out, and it removes that acquired character. Without it the system's own storage undoes some of its work.

Why it is different from a pre filter

A pre filter is protecting something expensive downstream, so it is sized for chlorine capacity and sediment load and changed on that basis. A post filter is protecting nothing: its whole job is the final taste, and it sees water that has already had almost everything removed, so it works very lightly. That is why it is small and why it can last a long time in theory. It is also why its failure has no consequence beyond taste, which is exactly why it gets forgotten.

The one that gets left in too long

Because a spent post filter causes no damage, nothing forces the issue and these routinely stay in for years. Two things happen. Carbon that has sat wet and inactive becomes a bacterial habitat, since there is no disinfectant residual in reverse osmosis water to suppress growth. And it stops doing the taste job, so an owner concludes the membrane has failed and replaces the expensive part while the actual cause sits at the outlet. If the water has gone stale, change this first.

Where else it appears

The same idea shows up at the end of whole house trains: a fine carbon stage after a backwashing media tank, catching fines carried out of the bed and finishing the taste. And it appears after ultraviolet on some designs, though carbon after UV is worth thinking about, since it removes nothing biological and adds a surface for growth downstream of the disinfection.
