Ultraviolet disinfection is highly effective within a narrow boundary and completely ineffective outside it, and the reason to be clear about the boundary is that a UV unit gives no indication of doing nothing. It runs, it shows a light, and it can be delivering almost no useful dose while the household assumes the water is protected.
What it does not remove
Everything except organisms. Sediment, hardness, dissolved iron and manganese, tannins, nitrate, arsenic, fluoride, lead, pesticides, chlorine and total dissolved solids all leave the chamber exactly as they entered it. There is no taste or odor change. Where any of those is the actual complaint, a UV unit is money spent on the wrong problem, and it is regularly sold to households whose water is unpleasant rather than unsafe. Purification here means microbiological only.
Where it fails at its own job
In water it cannot see through. Suspended particles shadow organisms from the lamp, and dissolved iron, manganese, tannins and hardness absorb or scatter the wavelength, cutting the delivered dose while the unit continues to appear normal. Flow above the rating cuts the dose too, since the water spends less time in the chamber. And an ageing lamp loses output steadily through the year while still glowing visibly. All four failures are silent, which is the recurring theme.
What it cannot protect
Anything downstream. There is no residual, unlike chlorination, so water that is disinfected at the unit can be recontaminated in a storage tank, in a dead leg, at a cross connection or by anything entering the plumbing after the chamber. That is why the unit goes last in the train and as close to distribution as practical, and why commissioning includes sanitizing the pipework downstream. A UV unit protecting contaminated plumbing is disinfecting water on its way into the problem.
Making it honest
Filtration ahead of it sized for the water, so the light reaches what it needs to. A unit rated for peak simultaneous flow rather than average. A UV intensity monitor reporting the light actually reaching the water, and ideally a solenoid that shuts the supply off when the dose falls below specification. An annual lamp change on the calendar and a scheduled sleeve clean. And periodic bacteriological testing, because that is the only thing that closes the loop.
